Please respond by Monday (the week of CAB) at the latest so transportation and lunch orders can be coordinated. If you do not confirm attendance, you are still more than welcome to join, however a meal will be subject to availability. Transportation will need to be confirmed by that Monday as well.

Review of New Patient Survey: We have revised the patient survey and would like your feedback on the questions.
Client Engagement: We need your ideas on how the agency can improve client engagement with labs and appointments. What are your suggestions?
